Professional Referral Form

Lighthouse is currently at referral capacity. Lighthouse cannot action a referral submitted for at least 4 working weeks. Please inform your client of this and consider other referral agencies. If you proceed with a referral, please inform your client, they will not be contacted for at least 4 working weeks. Additionally, as a referrer, please manage any client risk until Lighthouse contacts the referrer with a referral outcome.

I, the referrer, confirm I have made the client aware they will not be contacted by Lighthouse for at least 4 working weeks. I, the referrer, confirm I will manage any client risks if applicable, until I the referrer, have been contacted by Lighthouse with a referral outcome.
